Important reminder:

Before starting your application, make sure you have everything prepared. The form cannot be saved mid-process, so it’s best to gather all your information beforehand.
 
Application Checklist:

☑  A recent photo of yourself.
☑  Passport details and a scanned copy (for non-U.S. citizens).
☑  Medication information (if applicable).
☑  Contact details for two emergency contacts.
☑ Credit card information (a $50 application fee will be charged upon submission).

Next Steps After Submission (If Approved):
  1. References: Provide one personal and one professional reference.
  2. Interview: Schedule a live interview with the Program Director.
